Job summary

Communicare is seeking a dedicated Case Manager to provide comprehensive Community Support Services (CSS). You will manage a caseload, assess client needs, and develop treatment plans while coordinating with the CSS Director and other agencies to ensure seamless, client-centered care.

Role requires a Bachelors degree in a related field, strong interpersonal skills, and a valid drivers license with a clean record. Travel to client homes and sites is expected as part of this role.

Qualifications

• Bachelors degree in social work, counseling, psychology, or related field.
• Ability to relate effectively to severely mentally ill individuals.
• Must have transportation and a valid drivers license with insurance and a safe driving record.
• Communicare insurable driving record required.

Responsibilities

• Provide case management services for CSS clients.
• Assess clients needs and develop appropriate treatment plans.
• Coordinate case management and health center services with other agencies.
• Provide supportive counseling to clients and families as needed.
• Assist CSS clients in housing applications and benefits.
• Assist clients with medications and medication management planning.
• Ensure access to mental health and community services; perform home visits as needed.
• Maintain documentation and complete reports and progress notes daily in the EMR.
• Attend meetings and trainings designated by CSS leadership and participate in staffings.
• Support grant activities and reporting; coordinate with treatment teams and billing.
• Perform other duties as assigned.
