Job summary

Meals on Wheels People seeks a part-time Client Services Specialist to conduct home-delivered client eligibility assessments and act as a liaison among clients, partners, and staff. The role involves in-home and remote assessments, data management, and resource connections to support homebound clients.

The ideal candidate provides effective communication, handles escalations professionally, and can travel across Multnomah, Washington, and Clark Counties when required.

Qualifications

• Associate Degree or equivalent in Gerontology, Counseling, Human Services, or Communication.
• Bilingual abilities may qualify for bilingual pay differential.
• Proficient in MS Office and client data systems.

Responsibilities

• Serve as liaison between partners, clients, and staff.
• Conduct eligibility screenings for home-delivered clients.
• Notify appropriate parties of new starts and changes promptly.
• Communicate with case managers, clients, and staff regarding authorizations.
• Handle escalated client concerns in a professional manner.
• Maintain data integrity in the client data management system.

They will serve as liaison between contract partners, participants, and staff to ensure accurate authorization and delivery of services. They will also provide in-home and remote needs assessments and resource connections to homebound clients.Principal Duties and Responsibilities (Essential Functions)Serve as a liaison between referring partners, contract partners, clients, and staffProvides direct one-on-one and phone screening with clientsNotify appropriate parties of new starts and changes promptlyCommunicates with case managers, clients, and staff regarding the status of authorizations and potential disruptions of servicesProvides guidance and support regarding meals served to both Meals on Wheels People staff, clients, and referring partnersHandles escalated client concerns and comments in a constructive, professional, and helpful mannerEnsures integrity of data in client data management system; updates client information as needed, and corrects inaccurate information as necessaryConduct home-delivered meal eligibility assessments annually in accordance with the Meals on Wheels People Operating Procedures.Assess clients met and unmet needs, existing support, and resources to develop an individual service plan:Provide information and referral to enable clients to access services on their ownContact resources and make referrals if the client is unable to access services on the client's behalfAssist in reducing barriers to serviceOther duties as assignedEducation & KnowledgeAssociate Degree or equivalent job experience in Gerontology, Counseling, Human Services, or CommunicationKnowledge of senior services and resources preferredSkills, Abilities & ExperienceBilingual candidates speaking a second language in Spanish, Russian, Chinese, Korean, Vietnamese or other highly used language may qualify for an additional $1.00 per hour bilingual pay differential.Proficient in computer applications and MS OfficeAbility to manage time effectivelyAbility to be flexible as new tasks ariseAbility to quickly learn new computer programs; experience with client management systems a plusGood recordkeeping and organizational skillsAbility to communication with diverse populationsAny satisfactory combination of experience, training and/or education which ensures the ability to perform the work may substitute for the above qualifications.Working Conditions/Physical DemandsFast paced, with significant people interactionDexterity of hands and fingers to operate a computer keyboard, mouse, 10 key and phoneSitting for extended periods of timeAbility to climb stairs to complete client visitsMinimal reaching, bending and stoopingValid driver's license, auto insurance and personal transportation requiredFrequent Travel throughout Multnomah, Washington and Clark Counties required when COVID-19 restrictions are lifted.Candidate must successfully pass the Meals on Wheels People pre-employment drug & alcohol test and criminal background checkAll applications must be completely fully (including references) in order to be reviewed by staff members.
