Responsibilities

• Manage daily and weekly schedules for clients and caregivers, ensuring visits are staffed and service gaps are addressed quickly.
• Serve as a primary point of contact for clients and families, answering questions, resolving concerns, and supporting a strong client experience.
• Partner with caregivers, clinical team members, and office leadership to support service delivery and continuity of care.
• Coordinate referrals, intakes, and start-of-care activity by gathering information and collaborating with internal partners.
• Maintain accurate client documentation and use office systems to support scheduling and reporting.
• Build and manage a caseload with focus on service consistency, responsiveness, and office performance.
• Respond to last-minute changes, call-outs, and urgent staffing needs.
