Qualifications

• Bachelor's Degree in Social Work with State of Michigan Licensure.
• One year of experience with adults with mental illness or related populations preferred.
• Master's Degree in Social Work, Psychology, or Counseling with State of Michigan Licensure.

Responsibilities

• Assess consumer needs and coordinate services through PCP and IPOS.
• Maintain therapeutic relationship and support recovery, self-determination.
• Link consumers with external resources and provide intensive case management when assigned.
• Provide crisis support services as needed.

Bachelor's Degree in Social Work (Michigan licensure)
