Qualifications

• Bachelor’s degree in social work is required.
• One year supervised experience with the target population is required; CAFAS training preferred.
• Appropriate licensure/certification and state credentials may be required.

Responsibilities

• Assess consumer needs and coordinate services through PCP/IPOS.
• Lead Family/Person Centered Planning meetings and develop IPOS/FPOS.
• Document progress notes and keep master caseload lists up to date.
• Provide transportation and link beneficiaries to needed supports and benefits.
• Coordinate referrals with CMHA staff and community agencies.
