Job summary

The role is based at the Florence outpatient office with travel to rural counties; reliable transportation is required. Hours are typically Monday through Friday, 8:30 am to 5:00 pm, with a strong team environment and comprehensive benefits.

Qualifications

• High school diploma or GED required.
• Experience in client outreach or social services preferred.
• Knowledge of confidentiality and HIPAA practices.

Responsibilities

• Assist clients to achieve identified goals and track engagement.
• Conduct outreach to encourage service engagement and identify barriers.
• Maintain chart compliance and complete required documentation.
• Attend supervision and case conferences as needed and keep admin informed.
