Job summary

NorthKey Community Care in Covington, KY is seeking a Lead Nurse Care Coordinator to oversee nursing staff, coordinate patient care, and collaborate with multidisciplinary teams to improve outcomes. The role emphasizes leadership, clinical excellence, and adherence to nursing standards. Work hours are generally Monday–Friday, 8:30 a.m.

Qualifications

  • Licensed RN in Kentucky with appropriate education.
  • Knowledge, training, and experience in behavioral health.
  • Strong written and verbal communication skills.

Responsibilities

  • Provide oversight, guidance, direction, and training to nurses, medical assistants, and care coordination staff in behavioral health, primary care, and care coordination.
  • Coordinate coverage of nurse, medical assistant, and care coordination staff to ensure client needs are addressed.
  • Provide input to program development and program evaluation.
  • Provide current and up-to-date information to Lead Medical Officer, CCBHC Project Director, and Chief of Integrated Care.
  • Monitor compliance of all nurses, medical assistants, and care coordination staff with regulations, requirements, and standards of care.
  • Engage clients diagnosed with SMI, SED, and SUD in a structured, collaborative relationship that empowers them to gain necessary skills in recovery and self-management of their mental health, substance abuse, or developmental disability.
  • Promote environmental and psychosocial health and maximize independence to improve lifelong well-being and functional status.
  • Provide education to clients to promote improved functioning skills and development.
  • Complete requests for prior authorizations as indicated to provide necessary services.
  • Participate in staffing meetings and consultations with other clinicians to assess care coordination needs of clients and encourage referrals.
  • Act as an advocate for clients to ensure they receive necessary services in the community.
  • Develop knowledge and establish good working relationships with community support resources and services within the community.
  • Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community.
  • Complete timely and accurate documentation within an electronic health record and other documentation required by the position.
  • Attend continuing education and in-service training to improve professional competency.
  • Maintain flexible hours to meet agency and client needs.
  • Perform evaluations of staff and assist in hiring new staff.
  • Other duties as assigned.

  • Effectively address the functional challenges of individuals served.
  • Identify and address risks and current or potential gaps in care to prevent unnecessary deterioration that could result in hospitalizations, emergency room visits, or loss of independence in vocational, academic, and/or social functioning.
  • Provide education to clients to promote improved functioning skills and development.
  • Complete requests for prior authorizations as indicated to provide necessary services.
  • Participate in staffing meetings and consultations with other clinicians to assess care coordination needs of clients and encourage appropriate referrals to internal and external community services.
  • Act as an advocate for clients to ensure they receive necessary services in the community.
  • Develop knowledge and establish good working relationships with community support resources and services within the community.
  • Maintain communication and positive multi-disciplinary working relationships with clinicians and other related professionals in the community. This will include making appropriate referrals, sharing clinical information within HIPAA guidelines, and providing clinically appropriate interventions and collaborations.
  • Complete timely and accurate documentation within an electronic health record and other documentation required by the position.
  • Attend and participate in relevant continuing education and in-service training to improve professional competency and clinical intervention skills.
  • Maintain flexible hours to meet agency and client needs.
  • Perform evaluations of staff and assist in hiring new staff.
  • Other duties as assigned.
