Responsibilities

• Acts as initial point of contact for department calls and triages calls appropriately.
• Provides administrative support for non–PACE care coordination functions.
• Arranges discharge transport and follow-up appointments; coordinates with SNFs and case managers.
• Documents participant concerns and facilitates resolution to ensure quality care.
