Eastern Nebraska Office on Aging - ENOA is seeking a Services Coordinator to determine long-term care needs and coordinate home and community-based services for eligible clients over 65. You will identify strengths and needs, develop collaborative service plans, and ensure quality care while maintaining HIPAA and regulatory standards.

Qualifications

• Bachelor's degree in human services or related field and RN licensure in Nebraska.
• Minimum two years of professional experience in long-term care, gerontology, rehabilitation, or health care management.

Responsibilities

• Report to work as scheduled on a regular and reliable basis.
• Complete required pre-service orientation and training.
• Maintain confidentiality per HIPAA regulations.
• Understand and comply with applicable laws, rules, and policies.
• Communicate clearly with diverse stakeholders to support client needs.
• Be a strong team player with positive attitude toward clients and staff.
• Demonstrate computer literacy and office software proficiency.
• Follow service coordination standards and professional discipline.
• Know Nebraska Medicaid program regulations and related health care requirements.
• Assess client strengths, needs, and safety; conduct home interviews and observations.
• Develop participant-centered service plans with measurable outcomes.
• Coordinate care with formal and informal supports and providers.
