Right at Home East Bay is seeking a caring, organized professional to own the client experience—from initial conversations and in-home assessments through ongoing care, caregiver support, and quality oversight. This client-facing role blends relationship-building with problem-solving to ensure high-quality, reliable home care across our service area.

The position offers a path from Care Coordinator to Care Manager, with a focus on growing leadership, improving care quality, and meeting KPI

Qualifikationen

• 2+ years in care coordination, client services, or home care.
• Excellent verbal and written communication.
• Experience with WellSky and Zoho CRM.
• Resides in service area; able to work onsite and drive across territory.
• Valid driver’s license and insured vehicle; on-call.
• HCA Registry or ability to complete LiveScan.

Aufgaben

• Conduct in-home consultations, assessments, and caregiver supervisory visits.
• Nurture relationships with clients, families, and care partners.
• Build and review individualized care plans with caregivers.
• Coach caregivers and serve as primary contact for families.
• Document visits and updates in WellSky and company systems.
• Prepare client binders and maintain records.
