Job summary

Responsibilities

• Create and manage care plans and client/family experiences.
• Develops and maintains the client care plan, communicating with clients, families, care partners and referral sources.
• Interfacing with clients and families to ensure safe, appropriate and meaningful care is provided.
• Mentor to caregivers and support their care delivery.
• Maintain a calm, positive demeanor and multitask in a high functioning office environment.
• Ideal candidate has prior experience in home care or medical office setting.
