Job summary

Life’s At Home Care in Virginia Beach is seeking a Client Care Coordinator to help new clients start strong and stay connected with families and caregivers to ensure care is going smoothly.

This 40-hour-per-week role is primarily office-based with about 25% local fieldwork, including travel; you’ll coordinate admissions, caregiver matching, and quality visits while supporting RN and scheduler to keep client records current.

Qualifications

• Experience in care coordination, client service, or case management.
• CNA experience with strong office and coordination skills is welcome.
• Professional communication, good judgment, and dependable follow-through.
• Valid driver's license and reliable transportation.
• Ability to meet background screening requirements.

Responsibilities

• Coordinate admissions, caregiver matching, and first-shift introductions.
• Check in with clients and caregivers, resolve concerns, and follow through on solutions.
• Make targeted quality visits and plan routes to reduce driving.
• Support caregiver orientation and coaching with the RN, and present caregiver recognition awards.
• Keep WellSky records current and coordinate with the Branch Manager, RN, and scheduler.

What we’re looking for

• Care coordination, client service, case management, or caregiver supervision experience.
• Social work or human services degree preferred; experienced CNAs with strong office and coordination skills welcome.
• Professional communication, sound judgment, discretion, and dependable follow-through.
• Strong computer and organizational skills; valid driver’s license, reliable transportation, acceptable driving record, and ability to meet background screening requirements.
