Job summary

Life’s At Home Care in Virginia Beach is seeking a Client Care Coordinator to help newly admitted clients start strong and stay connected with families and caregivers. You will coordinate admissions, caregiver matching, and first-shift introductions while supporting ongoing communication and issue resolution.

The role involves 40 hours per week, predominantly in-office with about 25% local travel to client sites.

Qualifications

• Experience in care coordination, client service, case management, or caregiver supervision.
• Social work or human services degree is preferred; CNAs with coordination skills welcome.
• Proven communication, judgment, discretion, and dependable follow-through.

Responsibilities

• Coordinate admissions, caregiver matching, and first-shift introductions.
• Check in with clients/caregivers, resolve concerns, and follow through on solutions.
• Make targeted quality visits and manage nearby stops to reduce driving.
• Support caregiver orientation and coaching with RN and acknowledge staff achievements.
• Keep WellSky records current and coordinate with Branch Manager and scheduler.

This is a 40-hour-per-week role, primarily based in our office with about 25% local fieldwork, including travel. Field time may vary with new admissions and client needs.

What you’ll do

• Coordinate admissions, caregiver matching, and in-person first-shift introductions.
• Check in with clients and caregivers, resolve concerns, and follow through on solutions.
• Make targeted quality visits and group nearby stops to reduce driving.
• Support caregiver orientation and coaching with the RN, and present caregiver recognition awards.
• Keep WellSky records current and coordinate with the Branch Manager, RN, and scheduler.

What we’re looking for

• Care coordination, client service, case management, or caregiver supervision experience.
• Social work or human services degree preferred; experienced CNAs with strong office and coordination skills welcome.
• Professional communication, sound judgment, discretion, and dependable follow-through.
• Strong computer and organizational skills; valid driver’s license, reliable transportation, acceptable driving record, and ability to meet background screening requirements.
