Qualifications

• Customer service and communication skills with attention to detail.
• Proficiency in computers and generating reports.
• Fluent in English; Spanish is a plus.
• Bachelor's degree; 2 years supervisory experience in home healthcare/medical setting.

Responsibilities

• Be the first point of contact for clients, partners, and care providers, delivering excellent customer service.
• Takes inquiries from potential clients, provides information on services and fees, and gathers relevant data for care planning.
• Monitor daily dashboard and coordinate with caregivers and clients.
• Share on-call responsibilities on a rotating schedule.
• Input care plans, ensure staffing, and review information with caregivers prior to start of care.
• Complete monthly client calls and document in the client file.
• Update care plans, upload monthly reports, and communicate changes to caregivers.
• Ensure best caregiver-client matches and compliance with care plans.
• Participate in staff training and meetings and perform clerical duties as needed.
