Health at Home in Port St. Lucie, FL is seeking a Client Care Consultant to connect with families, educate them on care options, and help them move forward with Home Health or Private Duty services.

The role blends healthcare, customer service, and sales, centered on building trust and guiding families through available options. You will coordinate consultations, assess patients, and admit them as needed while collaborating with clinical, intake, scheduling, marketing, and leadership teams to

Formación

• Sales, admissions, intake, customer service, or healthcare experience.
• Home Health, Private Duty, hospice, senior living, healthcare, or related industry experience preferred.
• Experience using a CRM or managing a sales pipeline is a plus.
• Strong communication, relationship-building, and organizational skills.

Responsabilidades

• Respond to inbound calls, website inquiries, and referral opportunities.
• Guide families through available care options and service pathways.
• Coordinate consultations, assessments, and admissions.
• Build trust and confidence with patients, families, and referral sources.
• Manage an active pipeline of opportunities and follow-up activities.
• Collaborate with clinical, intake, scheduling, marketing, and leadership teams.
• Occasionally meet with families in homes, hospitals, rehabilitation facilities, and senior living communities.
• Help more patients access the care they need while supporting the agency's growth goals.
