Job summary

Home Instead is seeking a Client Care Advisor to serve clients and families in Summit and Medina Counties. This in-person role runs Monday through Friday, 8:30 am to 4:30 pm, with a base salary of $50,000–$60,000 plus bonus opportunities.

You will own relationships from day one, assess needs, build care plans, coordinate with schedulers and healthcare professionals, and ensure quality visits with confidentiality and integrity in the community.

Qualifications

• Experience in client services, healthcare, senior care, social work, or care coordination (3+ years).
• Two years of care management experience with writing and editing plans of care.
• College degree preferred.
• Microsoft Office proficiency; CRM/client management software a plus.
• Valid driver's license and reliable transportation.
• Availability for some evenings/weekends.
• High integrity and confidentiality.

Responsibilities

• Own the relationship as the primary contact for clients and families from day one.
• Think fast and solve faster — handle concerns with empathy and quick judgment.
• Consult and close — run care consultations and build custom care plans.
• Keep it moving — coordinate with schedulers, Care Pros, and healthcare pros.
• Follow through — quality assurance visits, documentation, and follow-up.
• Spot what's next — flag when a care plan needs to evolve.

What You'll Need

• Client services, healthcare, senior care, social work, or care coordination experience, at least 3 years
• Do you have at least two (2) years of care management experience? I.E. writing, editing plans of care?
• College degree preferred
• Microsoft Office proficiency; CRM/client management software a plus
• Valid driver's license, current car insurance and reliable transportation
• Availability for some evenings/weekends
• High integrity and the ability to keep things confidential
