Job summary

Home Instead in Ohio is seeking a Client Care Coordinator in an in-person role. You will be the go-to contact for clients and families, coordinating care plans and ensuring smooth operations across schedules and healthcare professionals.

The position offers a base salary of $50,000–$60,000 per year, plus bonus opportunities, with full benefits and on-call rotation compensation. This role emphasizes reliability, empathy, and positive community impact.

Qualifications

• 3+ years in client services, healthcare, senior care, social work, or care coordination.
• College degree preferred but not required in all cases.

Responsibilities

• Own the relationship as the primary contact for clients and families from day one.
• Respond empathetically to concerns and provide quick, practical solutions.
• Conduct care consultations, assess needs, and build care plans.
• Coordinate with schedulers, Care Pros, and healthcare professionals to keep care on track.
• Handle quality assurance visits, documentation and follow-up.
• Identify when a care plan needs updating and advocate for changes.

The Job

You're the go-to person for clients and families — the one who builds trust fast, solves problems on the spot, and keeps every moving piece of care on track.

• Own the relationship. Be the primary point of contact for clients and families from day one.
• Think fast, solve faster. Handle client concerns with empathy and quick, practical judgment.
• Consult and close. Run care consultations, assess needs, and build custom care plans.
• Keep it moving. Coordinate with schedulers, Care Pros, and healthcare pros so nothing slips.
• Follow through. Own quality assurance visits, documentation, and follow-up — no dropped balls.
• Spot what's next. Flag when a care plan needs to evolve and advocate for it.

What You'll Need

• Client services, healthcare, senior care, social work, or care coordination experience, at least 3 years
• Do you have at least two (2) years of care management experience? I.E. writing, editing plans of care?
• College degree preferred
• Microsoft Office proficiency; CRM/client management software a plus
• Valid driver's license, current car insurance and reliable transportation
• Availability for some evenings/weekends
• High integrity and the ability to keep things confidential
