Qualifications

• Bachelor’s degree in social service or related field.
• Arizona licensure/certification in social work or psychology is preferred.
• Experience with intake processes and behavioral health settings.
• One year of experience with behavioral health clients preferred.
• CPR/AED certification is required or obtainable.
• Fingerprint clearance card required within 30 days of employment.

Responsibilities

• Provide intake for participants in the Behavioral Health Program using required forms.
• Conduct telephonic and initial in-person intake assessments, referrals, and admissions.
• Ensure prompt response to client and referral needs.
• Provide clinical screening and risk assessment with appropriate oversight.
• Maintain documentation, records, and required reports.
• Participate in staff meetings and submit monthly activity reports.
• Promote Adelante Healthcare’s patient-centered care philosophy.
• Support IPS procedures during intake.

Education

Bachelor’s Degree in Social Service field

Current Arizona Certification as an LCSW, LPC, LISAC or PsyD

Associate/BHT level staff with intake training or two years’ experience

Minimum of one (1) year of experience working with Behavioral Health clients

Certification to perform CPR for Health Care Professional (CPR) and AED

Valid Level One Fingerprint Clearance Card (AZ DPS)
