Qualifications

• Bachelor's degree in Education, Social Work, Counseling, Psychology, Human Services, Sociology, or related field.
• Criminal background check conducted pursuant to WV Code §18A-3-10.

Responsibilities

• Implement the Communities In Schools Integrated Student Supports model at assigned school locations.
• Lead a School Support Team including administrators, teachers, counselors, and families.
• Conduct annual needs assessments using attendance, academic, and behavior data.
• Develop and monitor a School Support Plan aligned with improvement goals.
• Identify at-risk students and coordinate Tier I supports to improve engagement and attendance.
• Coordinate Tier II interventions for groups needing additional support.
• Provide ongoing case management with goal setting and progress monitoring.
• Maintain confidentiality and regular communication with parents and guardians.
• Coordinate referrals to community resources and services.
• Collect and report program outcomes and maintain CISDM data entries.

Education
