Responsibilities

• Act as liaison between health system, social services, and the community.

• Provide longitudinal support across the episode of care to promote trust and connection to services.
• Document data in the electronic medical record to support the care plan.
• Collaborate with an interdisciplinary care team in outpatient clinics and the community.
