Responsibilities

• Act as liaison between the health system, social services, and the community.
• Advocate for patients to ensure access to necessary care and resources.
• Support patients across the care episode to promote trust and continuity.
• Develop relationships to provide longitudinal support and social stability.
• Collaborate with interdisciplinary teams in outpatient clinics, community, and telephonic support.
• Assist individuals and families to access NM's social determinants of health resources.
• Document data in the EMR to support the care plan.
• Participate in professional development and orientation of new CHWs.
• Perform other administrative duties as needed.
