Job summary

Your Health in Bennettsville, SC is seeking a dedicated Community Health Worker (CHW) to support patients at the Bennettsville clinic and in homes and ALFs. The CHW will liaise with care teams, coordinate visits, and help ensure timely access to medications and DME.

role includes daily travel, vehicle use, and 12-hour shifts from 7:00 AM to 7:00 PM, with opportunities for growth within a leading physician group serving SC and GA.

Qualifications

• Certified Medical Assistant, Paramedic, or Licensed Nurse preferred.
• Phlebotomy experience preferred; willing to train if not experienced.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology tools for remote monitoring, telemedicine, and EMR.

Responsibilities

• Work with care team to ensure coordinated patient care per care plan.
• Communicate effectively by phone, email, Teams, and in person.
• Coordinate visits with PTs, OTs and Providers in homes and ALFs.
• Deliver medications to patients' homes, review instructions, and reconcile.
• Coordinate telehealth visits between patients and pharmacists, specialists, therapists, and PCPs.
• Set up training on Remote Patient Monitoring (RPM) equipment and provide ongoing support.
• Deliver DME to homes and ALFs, with training on correct use.
• Perform venipuncture for lab testing and proper specimen handling.
• Collect blood specimens during home/facility visits as needed.
