SuperCare Health is seeking a Telecare Care Coach for a hybrid in-office role (2-3 days on-site) to support Chronic Care Management programs. You will handle patient data entry, scheduling of RT visits, and coordination with care teams to ensure timely services and documentation.

The ideal candidate has a High School diploma and 3 years in customer service within managed care, plus proficiency with MS Office. Strong communication and organizational skills are essential for success.

Formación

• High School diploma required.
• Three (3) years’ experience of customer service or transaction processing in a Managed Care setting.
• Medicare Advantage knowledge of enrollment activities preferred.
• Working knowledge of MS Office including Word, Excel, and Access with proficient typing skills.

Responsabilidades

• Enter new patient demographics into the database and prepare delivery kits.
• Schedule initial Respiratory Therapist home visit, appointments and equipment delivery.
• Maintain census and access member portals to retrieve information and documents.
• Coordinate imaging and documentation per policy; ensure completeness and accuracy.
• Schedule meetings and record minutes for the Clinical Director and Interdisciplinary Care Team.
• Respond to inquiries from patients, families, and care team members via phone, email, or mail.
• Perform outreach calls to patients and caregivers and schedule in-system encounters or video visits.
• Maintain reporting, logs, and updated equipment records; ensure proper cleaning and return of equipment.
