Job summary

Center for Primary Care is seeking a Care Coordinator to support chronic care management across CPC locations. This role requires traveling to Martinez, GA for on-site training, then hybrid/remote work with monthly in-person meetings and bi-monthly travel to CPC offices.

You will conduct patient outreach, monitor health status, coordinate care with clinicians, maintain EMR documentation, and ensure HIPAA compliance.

Qualifications

• Education: High School graduate or GED required.
• Licensure/Certification: Registered MA, Certified MA, or current LPN required; Basic Life Support or ability to obtain within 6 months.
• Experience: 1-2 years of Medical Assistant or similar medical experience.

Responsibilities

• Coordinate and perform patient outreach and monitor health status.
• Review patient conditions and compliance with physician-directed care.
• Maintain accurate documentation in EMR and other databases.
• Provide effective communication between patients, medical staff, and providers.
• Assist in coordination of care with physicians, specialists, and insurance providers.
• Travel as required; on-site training at Martinez, GA, then hybrid/remote work.
