Job summary

New York Cancer & Blood Specialists is seeking a CNA/Medical Assistant to join the Chronic Care Management team in Bohemia, NY. You will introduce the CCM program to eligible patients, obtain verbal consent, and document in the EMR system.

Under supervision, you will review care plans, advocate for patients, and coordinate with wellness services. You will also facilitate communication between providers and patients, ensuring timely access to needed services and thorough documentation of all

Qualifications

• High school diploma or equivalent required.
• Certified CNA or Medical Assistant credential.
• Minimum 2 years of relevant experience.

Responsibilities

• Promote and introduce the CCM program to eligible patients via phone.
• Obtain verbal consent and document in the EMR system.
• Review and monitor patient care plans under CCM supervision.
• Advocate for patients and coordinate with wellness services.
