Qualifications

• CCM experience in a clinical setting is required or strongly preferred.

Responsibilities

• Assist providers in managing patients with chronic conditions such as diabetes, hypertension, COPD, and others.
• Perform patient intake including vital signs, med history updates, and medication reconciliation.
• Coordinate and document Chronic Care Management services per CCM guidelines.
• Educate patients on disease management, medication adherence, and lifestyle changes.
• Schedule follow-ups and facilitate communication between patients and providers.
• Maintain accurate patient records in the EHR system.
• Support care coordination to improve outcomes and reduce readmissions.
