Qualifications

• Active and unrestricted RN license.

Responsibilities

• Perform telephonic symptom assessment for patients, practice-wide.
• Utilize triage protocols to determine level of care and urgency.
• Provide patient education on symptom management and treatment side effects.
• Create and revise care plans for newly enrolled patients as clinically appropriate.
• Coordinate care with physicians, APPs, and staff.
• Maintain accurate documentation in compliance with HIPAA and regulations.
