Job summary

New York Cancer & Blood Specialists (NYCBS) is seeking a CNA/Medical Assistant for Chronic Care Management in Bohemia, NY. The role involves introducing CCM to eligible patients by phone, documenting care plans in the EMR, and coordinating with the care team.

Under CCM supervision, you will review documents, advocate for patients, and ensure access to services while maintaining accurate records. The position is full-time with daytime hours.

Qualifications

• High School Diploma or equivalent.
• Certified CNA or Medical Assistant.
• 2+ years of applicable experience.
• Bilingual English/Spanish a plus.
• Proficiency in Word/Excel.

Responsibilities

• Work within the Chronic Care Management team.
• Introduce the CCM program to eligible patients by telephone and obtain verbal consent.
• Document activities in the EMR system under CCM supervision.
• Advocate for patients and coordinate with wellness services.
• Facilitate information flow between providers and patients and access to services.
