Job summary

Spectrum Human is seeking a Children's Support Coordinator in Grand Rapids, MI to develop person-centered plans for children with developmental disabilities and coordinate services with families.

The role focuses on advocacy, outcomes monitoring, and building strong community connections. A Bachelor's in a Human Service field is required; Michigan social work licensure or eligibility is preferred, with a valid driver's license and 1+ year clinical experience recommended.

Qualifications

  • Bachelor's Degree in a Human Service-related field.
  • Licensure as Social Worker in Michigan or qualifications to obtain (preferred).
  • Valid Driver's License.
  • Minimum of one-year clinical experience with the population served (preferred).
  • Meets QIDP (Qualified Intellectual Disability Professional) qualifications (preferred).

Responsibilities

  • Assist children with developmental disabilities in building on their strengths.
  • Help families remain safely together in the family home.
  • Link, monitor, and coordinate services.
  • Develop and implement Person-Centered Plans for assigned individuals.
  • Advocate effectively for individuals served.
  • Monitor and document outcomes, revising plans as needed.

Children's Support Coordinator

Regular Full-Time WESTERN REGIONAL OFFICE, Grand Rapids, MI, US

Salary Range: $20.00 To $25.00 Hourly

Role and Responsibilities:

  • Assist children with developmental disabilities in building on their strengths
  • Help families remain safely together in the family home
  • Work in an environment convenient to the child and family
  • Link, monitor, and coordinate services
  • Develop and implement Person-Centered Plans for assigned individuals
  • Advocate effectively for individuals served
  • Monitor and document outcomes, revising plans as needed
  • Support culturally competent, recovery-based practices

Work Schedule and Benefits:
