  • Currently MichiCANS certified or ability to get certified upon hire.

Responsibilities

  • Develop, implement, and monitor IPOS individualized to the child’s strengths, needs, preferences, and goals.
  • Facilitate person-centered planning meetings with families, guardians, providers, schools, and partners to identify needs and develop effective strategies.
  • Serve as the primary coordination point ensuring services are delivered per approved plans.
  • Monitor progress toward goals, evaluate service effectiveness, and propose adjustments.
  • Advocate for families navigating behavioral health, education, healthcare, and community services.
  • Provide information about available resources and self-advocacy strategies.
  • Collaborate with providers and teams to address barriers and support positive outcomes.
  • Maintain timely documentation in EMR including progress notes, assessments, authorizations, and reports.
  • Participate in team meetings, training, quality improvements, and agency initiatives.
  • Perform other duties as assigned.

Summary

The Children's Intellectual and Developmental Disabilities (IDD) Supports Coordinator is responsible for coordinating services and supports that promote the health, independence, and overall well-being of children with developmental disabilities. This position works collaboratively with children, families, guardians, providers, schools, and community partners to develop and monitor person-centered plans that reflect each child's unique needs and goals. The Supports Coordinator serves as an advocate and resource for families while ensuring services are effectively coordinated, documented, and delivered in compliance with agency, state, and federal requirements.

Responsibilities
  • Develop, implement, and monitor Individual Plans of Service (IPOS) that are individualized to the child’s strengths, needs, preferences, and goals.
  • Facilitate person-centered planning meetings and collaborate with children, families, guardians, service providers, schools, and community partners to identify needs and develop effective support strategies.
  • Serve as the primary point of coordination for assigned children and families, ensuring services and resources are delivered effectively and in accordance with approved plans.
  • Monitor progress toward goals, evaluate service effectiveness, and make recommendations for adjustments as needed.
  • Advocate for children and families by assisting them in navigating behavioral health, educational, healthcare, and community service systems.
  • Provide information and education to children, families, and caregivers regarding available resources, benefits, community supports, and self-advocacy strategies.
  • Collaborate with providers and interdisciplinary teams to address barriers, resolve service concerns, and support positive outcomes for children served.
  • Maintain timely, accurate, and complete documentation in the Electronic Medical Record (EMR), including progress notes, assessments, service authorizations, and required reports.
  • Participate in team meetings, training, quality improvement activities, and agency initiatives as needed.
  • Perform other duties as assigned.
Qualifications
Education
  • Bachelor’s degree in social work or a related human services field.
Experience
  • Minimum of at least 1 year of experience working with children with developmental disabilities.
  • Supports coordination experience preferred for this position.
Credentials
  • QIDP credential or ability to obtain within 60 days of hire.
  • Currently MichiCANS certified or ability to get certified upon hire.
