Job summary

Brightwater Health in Cloquet, MN seeks a Community Support Provider to work with children 0–18 diagnosed with mental illness, coordinating services and delivering skill-building in home, school, clinic, and community.

You will collaborate with therapists and other care professionals, assist families in accessing services, and provide individual, group, and family training with flexible scheduling and some evenings; a comprehensive benefits package supports work-life balance.

Qualifications

  • Must meet the qualifications of a Mental Health Practitioner in Minnesota Statute 2451.04.
  • Bachelor's degree in human services or related field with 2,000 hours of mental health experience.
  • OR high school diploma with 6,000 hours of mental health experience.
  • Valid Driver's License and reliable vehicle with proof of insurance.
  • Ability to submit and pass a DMV/Criminal background check.
  • Strong interpersonal and cultural competency skills.

Responsibilities

  • Improve functioning of children in home, school and community.
  • Assist families in accessing services to meet mental health needs for ages 0‑18.
  • Collaborate with clinical team to create, review and implement treatment plan goals.

Summary:

Community Support Providers work with a caseload of children diagnosed with a mental illness to coordinate and provide services to them and their families. This position works to help the child and family to address skills deficits in the following areas: mental health, social skills, educational skills, vocational skills, and recreational skills. This position works closely with other community support providers, clinical staff and other care providers to help the client meet their identified goals. Services are provided in the clinic, community and client’s home.

Duties:
  • The program aims to improve the functioning of children in the home, in school, and in the community.
