Job summary

Brightwater Health in Cloquet, MN seeks a Community Support Provider to work with children 0–18 diagnosed with mental illness, coordinating services and delivering skill-building in home, school, clinic, and community.

You will collaborate with therapists and other care professionals, assist families in accessing services, and provide individual, group, and family training with flexible scheduling and some evenings; a comprehensive benefits package supports work-life balance.

Qualifications

• Must meet the qualifications of a Mental Health Practitioner in Minnesota Statute 2451.04.
• Bachelor's degree in human services or related field with 2,000 hours of mental health experience.
• OR high school diploma with 6,000 hours of mental health experience.
• Valid Driver's License and reliable vehicle with proof of insurance.
• Ability to submit and pass a DMV/Criminal background check.
• Strong interpersonal and cultural competency skills.

Responsibilities

• Improve functioning of children in home, school and community.
• Assist families in accessing services to meet mental health needs for ages 0‑18.
• Collaborate with clinical team to create, review and implement treatment plan goals.
• Provide individual, group and family skills training in home, school and community.
• Requires flexible scheduling and some evening work.
• Work with a multidisciplinary team including therapists, psychologists and child psychiatry.
• Knowledge of community resources and area providers.
• Effective time management and ability to be self-directed.
• Crisis intervention skills and employment-related skills knowledge.
• Experience with diverse populations and families facing homelessness or legal system involvement.
