Job summary

Brightwater Health is seeking a Care Coordination Program liaison in Duluth, MN. The role connects clients with Brightwater Health programs and outside providers to close gaps in care, supporting transitions and referrals while ensuring continuity of services.

Responsibilities include engaging clients, navigating services, documenting in the EHR, and participating in team case consultations. A focus on relationship-building and cultural competency is essential.

Qualifications

• Bachelor's degree in human services or related field with 2,000 hours of mental health experience.
• OR high school diploma with 6,000 hours of mental health experience.

• Maintain accurate, timely documentation in the electronic health record.

Summary:

The Care Coordination Program connects clients, Brightwater Health programs, and outside providers to close
the gaps between services, whether after a discharge, during a wait for an opening, or after a referral. Care
Coordinators engage assigned clients, help them navigate barriers and follow up until they are connected to
care. Working alongside existing department processes, they serve as the connective layer that holds a client's
care together, so no one falls through the cracks between services.

Duties:

• Reach out to assigned clients to begin Care Coordination services.
• Help clients navigate internal and external services.
• Coordinate care across Brightwater Health programs and external community resources to support the
  client's overall needs.
• Coordinate transitions for clients discharging from higher levels of care.

Minimum Qualifications:

• Bachelor's degree in human services or related field and 2,000 hours of mental health experience.
  OR high school diploma and 6,000 hours of mental health experience.
• Valid Driver's License and reliable vehicle with proof of insurance.
• Ability to submit and pass a DMV/Criminal background check.
  
• Strong interpersonal and cultural competency skills.
