Qualifications

• Bachelor's degree in human services or related field required.
• Experience in the human services field required.
• Ability to work independently and as part of a team.
• Completion of required training upon hire.
• Must have a dependable vehicle and valid driver’s license.
• Positive and effective communication with others.
• Basic math proficiency and office technology use.
• Requires use of a cell phone during work hours.

Responsibilities

• Develop therapeutic rapport with diverse individuals within the target population.
• Assess and function in a strengths-based model of care.
• Link patients with resources and coordinate appropriate treatment.
• Provide documentation for the clinical record.
• Meet productivity standards and participate in staff meetings.
• Maintain clinical documentation that reflects medical necessity and progress.
• Collaborate with peers and partner organizations to support patients.
