Qualifications

• BA/BS degree or equivalent experience in human services.

Responsibilities

• Develop continuous supportive relationships with identified clients and their families.
• Assist with targeted case management/care coordination duties as needed.
• Maintain direct client contacts in their environment; schedule needs per plan of care.
