Job summary

This position supports children in the PRP program, implementing rehabilitation plans and coordinating services with families and professionals. Responsibilities include intake, IRP development, home visits, group activities, and ensuring timely documentation to support program payments.

Qualifications

• Knowledge of DSM and child diagnoses.
• Ability to create and implement IRPs.
• Strong written, interpersonal, and oral communication skills.
• Valid Maryland driver’s license required.

Responsibilities

• Implement on-site and off-site rehabilitation activities per IRP.
• Conduct intake meetings for new clients.
• Develop and implement IRPs based on child needs.
• Conduct monthly in-home/community visits to support goals.
• Coordinate PRP group activity sessions and transport as needed.
