Job summary

Key Point Health Services is hiring a Child PRP Rehabilitation Counselor in Aberdeen, MD. The position focuses on implementing rehabilitation plans for children, conducting intake, and coordinating services across home and community settings.

Qualifications

• Knowledge of DSM and symptoms of common pediatric PRP diagnoses.
• Ability to create and implement Individual Rehabilitation Plans (IRP).
• Strong writing, interpersonal, and oral communication skills.
• Valid Maryland driver’s license required.

Responsibilities

• Implement on-site and off-site rehabilitation activities per IRP for assigned clients.
• Conduct intake meetings for children accepted to the PRP and complete required paperwork.
• Develop or implement IRPs based on each child’s strengths and needs.
• Complete home or community visits to help children reach identified goals.
• Lead PRP group activity sessions for enrolled children.
• Provide individualized case management and psychoeducation to implement goals.
• Arrange transportation to PRP activities as needed.
