Family & Children’s Services is seeking a compassionate Case Manager to support high-risk adults with severe mental illness. The role encompasses assisting with income supports, housing, transportation, and access to essential resources, along with crisis intervention and safety planning.

Services are delivered in person or via telehealth, with a focus on person-centered care and data-driven outcomes. You will work within a CC BCHC framework, coordinating care through multidisciplinary teams to

Qualifications

• Bachelor's Degree in Social Work or related field (required).
• Case Management II certification or obtain by ODMHSAS after hire.
• Must possess a valid Oklahoma Driver License and travel to locations other than primary office.

Responsabilités

• Provide services for identified needs including income, housing, transportation access, and other supports.
• Educate individuals or groups on money management, symptom education, and interpersonal skills.
• Provide crisis intervention and safety planning.
• Deliver services in-person or via telehealth as needed.
• Gather and complete all CCBHC data metrics.
