Job summary

Pitkin County Human Services is seeking a dedicated case manager to assess the safety and wellbeing of at‑risk children and to help families access essential community resources. The role includes intake assessments, plan development, and coordination of services to improve outcomes.

You will document all activities in state systems, ensure compliance with county, state, and federal requirements, and work with diverse populations.

Qualifications

Bachelor's Degree in Social Work, Counseling, Human Services, or related field.
Minimum 2 years of experience working with children and/or families per state statute.
Must possess a valid Colorado Driver's License and acceptable Motor Vehicle Record.
Level II: Child Welfare/Adult Protection Training Academy Certification.
Level III: 3+ years progressively responsible experience and 3+ years facilitating family meetings.

Responsibilities

Assess vulnerable families and children; evaluate needs, resources, barriers, and finances; perform intake assessment and develop plans.
Develop improvement plans and goals using human services theories, state laws, and community resources.
Refer clients to community services and coordinate multiple services.
Determine urgency and severity of risk; make recommendations on actions.
Document all assessments and follow-up in state tracking systems; maintain confidentiality.
