Responsibilities

• Provide case management services (information, plan referrals and linkages).
• Develop and maintain case records and service documentation.
• Document case activities in FSFN within 24 hours of events.
• Complete required court reports and testify as needed.
• Monitor quality of care with monthly home visits.
• Coordinate services with professionals and paraprofessionals.
• Conduct community outreach and provide transportation as needed.
• Perform monthly visits with parents in Out of Home Care on case progress.
• Refer families to services and interpret information from reports.
• Provide post-placement planning and support where applicable.
