Job summary

Claratel Behavioral Health seeks a Child & Adolescent Case Manager/Community Support Individual to deliver recovery-focused, family-driven services in the community. Under supervision, CSIs support treatment goals, coordinate care, attend meetings, and promote family navigation and natural supports.

Responsibilities include assessments, care coordination, ongoing case management, timely documentation, and collaboration with a multidisciplinary team to meet client goals.

Qualifications

• Bachelor’s degree in a related field (such as counseling, criminal justice, or social work)
• At least two years’ experience in a behavioral health setting providing case management/community support services
• Proficient in technology skills to be able to use electronic medical records, computers, telehealth systems, and cell phones.
• Care coordination skills to assist individuals with accessing/linking additional resources and/or services within the agency and/or community.
• Must have transportation to and from work, strong interpersonal skills & be a positive team player.
• Pre-employment drug screening may be required

Responsibilities

• Completes community needs and clinical assessments for all individuals assigned to caseload
• Is a member of interdisciplinary treatment team meetings to provide input for client treatment needs
• Provides ongoing support, case management, and skill-building functions to help the individual achieve goals
• Provides timely, high-quality documentation for all service contacts
• Meets regularly with the Program Manager for professional growth and administrative issues
• Outreach to all clients assigned to caseload at least twice monthly
• Facilitates communication between various community providers on behalf of the client
• Communicates effectively with clinicians/agency staff about challenges/concerns with clients
• Maintains ethical standards and communicates safety/issues to the Program Manager
• Responsible for transporting participants on community outings
• Additional duties as assigned by Program Manager/Director
