Job summary

The Posting Board, Inc. is seeking a full-time community-based case manager to support youth enrolled in Comprehensive Community Services (CCS) and Children’s Long-Term Support (CLTS) programs in Lincoln County.

You will assess needs, plan services, teach skills, and coordinate care to promote recovery, independence, and safety. Work with clients, families, treatment teams, and community providers; ensure documentation complies with state regulations.

Qualifications

• Bachelor’s degree in Human Services, Social Work, or a closely related field required (Master’s degree preferred).
• Minimum 1 year of direct case management experience required (3+ years of case management and 1+ year of CCS, CSP, CLTS, or CST preferred).
• Active Wisconsin driver’s license with an unrestricted driving record, approval to transport clients, and access to a personal vehicle required.

Responsibilities

• Assess and plan services using state-approved screenings; develop individualized service/treatment plans.
• Coordinate services across psychiatry, primary care, schools, courts, housing, and substance use providers; lead multidisciplinary meetings.
• Provide side-by-side skill instruction (daily living, budgeting, symptom management, social skills); assess risk and coordinate crisis plans.
• Discharge planning involvement and documentation in compliance with state regulations.
