Job summary

Synergy HomeCare in Illinois seeks an experienced Medicaid Referral Relations & Business Development Manager to expand our referral network across Chicago and surrounding suburbs. You will leverage established connections to drive sustained partner growth.

Ideal candidates have strong IDoA/CCU ties, MCO relationships, and a proven record in Medicaid home care referrals, with a focus on timely client onboarding and service coordination.

Qualifications

• Strong network with Illinois Medicaid stakeholders (IDoA, CCUs, MCOs).
• Proven track record in developing healthcare referral relationships.
• Deep understanding of Medicaid home care referral processes.
• Ability to schedule and conduct meetings with case managers.

Responsibilities

• Establish and strengthen relationships with IDoA, CCUs, MCOs, and Medicaid organizations.
• Develop a pipeline of Medicaid home care inquiries and referrals.
• Schedule meetings with case managers and referral partners.
• Promote services, caregiver availability, and coverage.
• Coordinate with admissions for timely client onboarding.
• Track outreach results and build long-term partnerships.
