Job summary

Saint John’s Communities is seeking an experienced Charge Nurse (RN) to provide comprehensive nursing care to residents and ensure satisfaction and policy adherence. You will assess residents’ needs, develop individualized plans, and collaborate with the care team to maintain high-quality service.

Responsibilities include supervising LPNs, Med Passers and RAs, documenting results, and leading teaching plans while ensuring compliance with HIPAA and safety procedures.

Qualifications

• Must be a graduate of an accredited School of Nursing and currently licensed as a Registered Nurse in the State of Wisconsin.
• Must have at least 2 years of experience in a geriatric/rehabilitation setting. One year acute medical/surgical hospital experience preferred.
• Must be CPR certified.
• Must be able to read, speak and write English.
• Knowledges of State and Federal regulations/guidelines for primary work area and the MDS system.

Responsibilities

• Utilizes the nursing process to perform comprehensive assessments of residents’ physiological, psychological, emotional, intellectual, vocational, and spiritual needs.
• Develops and revises individualized plans of care.
• Documents assessment results thoroughly, accurately, and timely.
• Collaborates with health care team via conferences, stand-ups, and resident/family meetings.
• Develops teaching plans reflecting residents’ learning styles.
• Acts as Charge Nurse and supervises LPNs, Med Passers, and RAs to ensure care is delivered as planned.
