Job summary

Priority Management Group, LLC is seeking a dedicated CNA for The Homestead of Denison in Denison, TX. You will provide compassionate resident care within the CNA scope, document care, and report changes under supervision, while supporting ADL tasks, vital signs, and meals.

Role includes trauma‑informed, person‑centered care for residents including those with dementia; participate in care planning and admissions/transfers; maintain certifications and follow infection prevention protocols.

Qualifications

• Current Texas CNA license/certification in good standing.
• Ability to provide age-appropriate care and follow standards of practice.
• Strong communication, teamwork and professionalism with residents, families, and staff.
• Comfort working with challenging behaviors and dementia/behavioral care.

Responsibilities

• Provide quality resident care within the CNA scope of practice and conduct assigned procedures under the direction of the Charge Nurse/Licensed Nurse Supervisor.
• Document care accurately and promptly (flow sheets, notes, charts).
• Report changes in condition to the Charge Nurse/Nurse Supervisor immediately.
• Provide ADL assistance per care plan, including hygiene, grooming and meal prep/transport.
• Take and document vital signs, weights, intake/output and meal consumption; report significant changes.
• Maintain resident comfort and safety; ensure call light is accessible.
• Support exams/tests/appointments and observe resident response to interventions/treatments.
• Provide trauma-informed, person-centered care, including residents with dementia or psychosocial needs.
• Participate in person-centered care planning and review care plans daily for changes.
• Communicate observations and concerns to nursing leadership per facility policy.
• Assist nursing with admissions, transfers and discharges; greet and orient new residents.
• Attend required orientation and ongoing education; maintain CNA certification; complete competencies.

• Support exams/tests/appointments as assigned and observe resident response to interventions/treatments

Care Plan and Assessment

• Participate in person-centered care planning and review care plans daily for changes
• Communicate observations and concerns to nursing leadership per facility policy

Admissions | Transfers | Discharges

• Assist nursing with admissions, transfers and discharges | Greet and orient new residents
• Introduce residents to roommates/staff and help inventory/label personal belongings

Staff Development

• Attend required orientation, in-services and ongoing education | Maintain current CNA certification
• Complete assigned competencies (e.g., resident rights, abuse prevention, dementia/behavioral care)

Safety | Sanitation

• Follow infection prevention, PPE and safety procedures | Keep call light within reach
• Clean/disinfect resident care equipment and shared devices | Conduct routine care-area housekeeping
• Report hazards, safety concerns, equipment issues and communicable disease concerns per policy
• Store tools/supplies appropriately and maintain required labels/SDS information

Equipment | Supplies

• Report defective equipment promptly | Follow lockout/tagout procedures

Qualifications: Education

Preferred

• High school diploma or equivalent

• Current Texas CNA license/certification in good standing

• 1+ year experience in a skilled nursing facility

Physical | Sensory Requirements

• Able to push, pull, move and/or lift a minimum of twenty-five pounds to a minimum height of five feet and be able to push, pull, move and/or carry such weight a minimum distance of fifty feet
• Assist in the evacuation of residents during emergency situations
