Job summary

Care Initiatives in Iowa is seeking a compassionate Licensed Practical Nurse (LPN) for a Charge Nurse - LPN role. You will provide comprehensive care and serve as clinical leader on the unit, coordinating resident care with the interdisciplinary team.

Available shifts include full-time and part-time weekend packages with competitive hourly rate and shift differential. You’ll supervise care staff, ensure regulatory compliance, and support residents and families.

Qualifications

• Valid Licensed Practical Nurse license in good standing. License must be valid within Iowa, or ability to activate.
• Current CPR Certification.
• Documented completion of State-approved LPN Supervisory Course or ability to obtain within first 90 days of employment.
• Knowledge of federal and state long-term care regulations and laws.
• Strong commitment to helping others with empathy, respect, patience, and discretion.

Responsibilities

• Conduct ongoing assessments of residents’ clinical status, including vital signs and condition changes, and contribute to individualized care planning with the interdisciplinary team.
• Deliver and oversee direct nursing care, including wound care, IV therapy (as permitted), catheter care, and other skilled interventions per physician orders and facility protocols.
• Administer medications and treatments safely, monitor therapeutic response and adverse effects, and ensure timely documentation.
• Serve as a clinical leader on the unit by coordinating resident care and communicating with physicians, nurse leadership, residents, and families.
• Supervise, mentor, and support CNAs and other staff, promoting accountability and high-quality resident care.
• Ensure adherence to state and federal regulations, infection prevention practices, and Care Initiatives policies.
• Provide education, guidance, and emotional support to residents and families, fostering trust and understanding of care plans.
