Job summary

Care Initiatives is seeking a compassionate Licensed Practical Nurse (LPN) to join our Charge Nurse team across Iowa's skilled nursing communities. Full-Time or Part-Time Evening shifts offer patient-centered care, mentorship, and leadership opportunities in a trusted family of facilities.

You’ll conduct assessments, administer medications, supervise CNAs, and collaborate with physicians to maintain high-quality, compliant resident care.

Qualifications

• Valid LPN license in good standing in Iowa.
• CPR certification is current.
• Completion of State-approved LPN Supervisory Course or ability to obtain within 90 days.
• Knowledge of long-term care regulations and best practices.
• A strong commitment to compassionate, patient-centered care.

Responsibilities

• Conduct ongoing assessments of residents’ clinical status and contribute to individualized care plans.
• Deliver and oversee direct nursing care, including wound care and IV therapy within scope.
• Administer medications and treatments, monitor response, and document findings.
• Serve as a clinical leader on the unit, coordinating resident care and communicating with the team.
• Supervise and support CNAs and other staff, promoting teamwork and high-quality care.
• Ensure adherence to regulations and infection prevention practices.
• Provide education and emotional support to residents and families about care plans.
