Job summary

Care Initiatives in Iowa is seeking a Charge Nurse - Licensed Practical Nurse (LPN) to provide comprehensive nursing care in our skilled nursing community. Full-time evening shift with every other weekend offers a meaningful opportunity to lead resident care, monitor conditions, and collaborate with the interdisciplinary team.

You will assess residents, administer medications, supervise CNAs, and ensure regulatory compliance while upholding compassion and dignity.

Qualifications

• Valid LPN license in good standing in Iowa.
• Current CPR certification.
• Ability to obtain Supervisory Course within first 90 days of employment.
• Knowledge of federal and state long-term care regulations.
• Empathy, respect, patience and discretion in care.

Responsibilities

• Conduct ongoing assessments of residents' clinical status and contribute to care planning with the interdisciplinary team.
• Deliver and oversee direct nursing care including wound care, IV therapy (as permitted), catheter care and other interventions per physician orders.
• Administer medications and treatments safely, monitoring responses and documenting accurately.
• Serve as a clinical leader on the unit, coordinating resident care and communicating with physicians, leadership, residents, and families.
• Supervise and support CNAs and other staff, promoting teamwork and high-quality resident care.
• Ensure adherence to regulations, infection prevention practices and facility policies to maintain safety and compliance.
• Provide education and emotional support to residents and families, fostering trust and understanding of care plans.
