Qualifications

• BSN required within four years of hire/promotion; education agreement may take precedence.
• Mentoring experience.

Responsibilities

• Performs assessment of physical, social, and psychological status.
• Collaborates with patient, family, physician, and team to develop plan of care.
• Directs interdisciplinary care team via delegation and coordination.
• Plans and prepares patients for further support needs after discharge.
• Manages staffing needs and resources for optimal patient care.
• Participates in continuous quality improvement and safety initiatives.
