Your Health Organization in Rock Hill, SC seeks a Nurse Practitioner to serve patients across care settings. You will evaluate high-risk patients, develop individualized care plans, and work with an interdisciplinary team to ensure optimal outcomes.

This full-time, salary-based position requires daily travel between offices and facilities, with opportunities for a monthly bonus. You will provide care in homes, facilities, telehealth, and clinics, delivering compassionate, patient-centered care.

Qualifications

• Must hold a valid Nurse Practitioner license in South Carolina.
• Experience delivering patient care in home health, clinics, facilities, telehealth, or specialty care settings preferred.
• Strong communication and collaboration within interdisciplinary teams.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate with case managers, coordinators, social workers, and specialists to optimize patient care.
• Participate in team meetings to discuss patient progress and treatment plans.
• Provide education to patients and families about conditions and self-management.
